Thank you for expressing an interest in volunteering at Northside Hospital-Forsyth. We welcome people who are available and willing to make a commitment to give their time and service to our community. All Northside Hospital-Forsyth volunteers are required to be members of the Auxiliary and attend an initial orientation, purchase a volunteer uniform to wear while on duty, have an annual TB skin test and pay annual dues. In addition, volunteers are required to work once a week for four hours at their assigned service.

Please print these forms and make sure you sign and date both pages. Then mail or deliver the application and background release forms to the Northside Hospital-Forsyth Auxiliary office, located at 1200 Northside Forsyth Drive, Cumming, Georgia 30041.
You also will need to schedule an interview appointment with Gloria Wyatt, coordinator of Volunteer Services, or one of the other officers of the Auxiliary. Someone will call you to set up a convenient time for an interview. Your application should be completed before the interview.

Orientations are scheduled for the third Wednesday of each month and last approximately four hours, followed by lunch in the hospital cafeteria. After orientation, each volunteer will be assigned to two four-hour shifts at the Hospital Transport Team Desk. This is a good way to get acquainted with the hospital’s many departments and a chance to meet other people. During this time, the Vice President of Orientation and Training will discuss possible volunteer assignments and later contact you either by phone or in person.
