Anyone who is scheduled to have a procedure performed should pre-register. This includes surgical procedures, labor and delivery and other procedures.

Pre-registering for your procedure shortens the admissions process and saves you time.

 

How do I pre-register? Back to top

Simply complete the online pre-registration form. You will receive a confirmation e-mail within 24 hours. This e-mail will contain a consent form. Please print this consent form and fax or

e-mail the consent form along with copies of the front and back of your insurance card(s).

How soon do I need to pre-register before my procedure? Back to top

As soon as possible. Please understand that forms submitted fewer than three days prior to your service date may not be processed in time.

How will I know if my pre-registration form has been received? Back to top

You will receive an email confirming that your pre-registration form has been received.

 

What do I do if some of my information has changed since I pre-registered? Back to top

When you present for your service or pre-testing, simply alert your clinician or registration representative about any changes. You may also fax changes to (404) 300-2333 or email them.
