National Healthcare Decisions Day

National Health Care Decisions Day (NHDD) happens in April and is a day used to inspire, educate and empower people to make their health care wishes known.

Advance care planning is the process of communicating your wishes for future health care to your loved ones and health care providers. It's important to have these conversations while you're still able to make decisions, so that your wishes are honored if you become unable to speak for yourself.

WHO: No matter your age, now is the time to make end-of-life health care decisions.

WHAT? National Healthcare Decisions Day (April 16) brings awareness to the public and health care professionals about planning and preparing for future health care decisions.

This special day is a reminder to schedule time to talk with family and friends about your choices and theirs for health care now and at the end of life.

WHY? 80% of Americans say they’d like to die at home. But more than 7 of every 10 Americans annually die in facility away from home.

HOW? Here are three steps to help you, your family or a friend prepare:
