Our team of exercise physiologists, nurses and medical directors prepares an individualized treatment plan after your initial assessment. PHASE II: We continue to monitor your heart rate, heart rhythm and blood pressure before, during and after exercise. Our cardiac rehab specialists will help you understand your heart function, the causes of heart disease and how to best manage your medications. Most importantly, we’ll teach you how to keep your heart strong through healthy lifestyle changes such as stress management and heart-healthy eating.
PHASE III: During the final phase of cardiac rehab, we support you as you regain strength and independence. We will still monitor your blood pressure and heart rate, but you can exercise without as much oversight. Quarterly progress reports are sent to your doctor so together you can discuss continued progress and new goals.
