  • Resources on managing heart disease risk factors such as blood pressure, stress, weight, cholesterol, and diabetes
  • Development of personal goals with ongoing measurement for continuous progress
  • Support for long-term disease management for improved quality of life
  • Referral to smoking cessation programs, if needed

Cardiac Rehab Services for Improved Healing 

For specialized help with healing and recovery, our cardiovascular team of certified rehab specialists at the Northside Hospital Heart Institute can help. You may be referred to the Cardiac Rehabilitation Program by your Northside Hospital doctor. You can also ask your doctor about our specially designed cardiac rehabilitation or pulmonary rehabilitation services for the following:

  • Angioplasty or stent placement
  • Cardiomyopathy
  • Chest pain (angina)
  • Coronary artery bypass surgery
  • Coronary artery disease
  • Heart attack
  • Heart failure and/or cardiomyopathy
  • Heart or lung transplant
  • Heart valve repair or replacement
  • Peripheral artery disease
  • Pulmonary hypertension

Our Whole-Person Approach to Wellness

Your health care team is an important part of your recovery. The Northside Hospital Cardiac Rehab Program provides medically supervised aerobic and strength training. Our team of exercise physiologists, nurses and medical directors prepares an individualized treatment plan after your initial assessment. Registered dieticians provide personalized dietary consultations and, together, we teach you ways to modify risk factors and improve your overall wellness. After this first phase of the program, our team provides ongoing support, including:

PHASE II: We continue to monitor your heart rate, heart rhythm and blood pressure before, during and after exercise. Our cardiac rehab specialists will help you understand your heart function, the causes of heart disease and how to best manage your medications. Most importantly, we’ll teach you how to keep your heart strong through healthy lifestyle changes such as stress management and heart-healthy eating.

PHASE III: During the final phase of cardiac rehab, we support you as you regain strength and independence. We will still monitor your blood pressure and heart rate, but you can exercise without as much oversight. Quarterly progress reports are sent to your doctor so together you can discuss continued progress and new goals.
